Intensive Case Management Teams (known as ICM or ICMT) provide a support role for people who experience severe substance use challenges, chronic unmet medical needs, and multiple barriers to accessing services.
Intensive Case Management Teams (known as ICM or ICMT) provide a support role for people who experience severe substance use challenges, chronic unmet medical needs, and multiple barriers to accessing services.
Clients must meet all the following:
- Reside on the North Shore
- 19 years or older
- Experiencing housing instability, at risk of homelessness, precariously housed and/or hard to reach
- Substance use concerns contributing to persistent and/or recurrent physical or psychosocial challenges
- Experiencing significant challenges requiring a coordinated, intensive approach to support their overall well-being
- Experiencing significant challenges accessing and remaining engaged with other services in community
- Aware of the referral and be willing to engage with the team
- Complex primary care needs
- Complex mental health needs (does not require diagnosis)
- Complex psychosocial needs
- Frequent use of hospital acute services

Hours of Operation: Monday and Saturday 8:00 AM to 4:00 PM, Tuesdays 8:00 AM to 6:00 PM and Wednesday to Friday 8:00 AM to 7:00 PM
Service is available in English.
Interpreter available upon request.
