Mental Health - Senior and Services for Seniors
Provided by Vancouver Coastal Health
Services include:
- Interdisciplinary assessment, treatment, and consultation
- Education to clients, their care partners, and community members
- Geri-psychiatric assessment and consultation
- Case management
- Psychosocial rehabilitation
- Functional assessment
- Group engagement
Eligibility:
- A progressive cognitive or memory disorder with associated psychiatric and/or behavior management concerns and/or cooccurring misuse of alcohol, prescription medications or other substances
- Recently developed serious mental illness and cognitive decline with physical or functional decline
- Severe and persistent mental illness with cognitive and physical or functional decline
