Older Adult Mental Health Program - North Shore

The Older Adult Mental Health Team offers services to help improve the quality of life of older adults (and their caregivers), who are vulnerable due to a combination of psychiatric, medical and social factors.
The program offers psychiatric assessment and treatment for older adults who may require outreach services. The approach is to match services with individual needs. The team provides case management and psychiatric services as needed and may also include liaising with pharmacies, families, and other community supports.
Services include:
  • Interdisciplinary assessment, treatment, and consultation
  • Education to clients, their care partners, and community members
Specialized services also include:
  • Geri-psychiatric assessment and consultation
  • Case management
  • Psychosocial rehabilitation
  • Functional assessment
  • Group engagement

Eligibility:
  • A progressive cognitive or memory disorder with associated psychiatric and/or behavior management concerns and/or cooccurring misuse of alcohol, prescription medications or other substances
  • Recently developed serious mental illness and cognitive decline with physical or functional decline
  • Severe and persistent mental illness with cognitive and physical or functional decline
