Vancouver Community Palliative Care Program - For Unattached Patients
End of Life Care / Palliative Care and Healthcare Providers

A pilot program whereby physicians provide specialized palliative care to patients living at home with life-limiting or life-threatening illnesses.
The existing program is a consultant model, and palliative care physicians work in collaboration with primary care providers (family physicians or nurse practitioners), home care nurses and allied health care providers to improve patient’s quality of life by addressing symptom management and suffering/distress.
As part of the Vancouver Community Palliative Care Program, a nurse practitioner is embedded in medical staff, to provide up to 3 months of short-term primary care for patients with palliative or end of life care needs who do not have a primary care provider. This will allow those without a primary care provider to receive timely, appropriate and integrated palliative and primary care at home as they approach end of life.
