Provides collaborative, interdisciplinary care for patients with chronic conditions
Chronic Disease Nurses (CDNs) work with physicians to provide collaborative, interdisciplinary care that patients with complex conditions often require in order to successfully achieve guideline-based targets and optimal health. Referred patients may be recently diagnosed or have long-standing conditions ranging from diabetes, COPD, hypertension, CHF, CAD, IHD, cancer, or stroke.
