- Brings together a support team of people to work with you such as your family, trusted friends, teachers and employment counsellors
- Serves as your single point of contact with government agencies and many community services
- Gives clear, reliable information about services and supports
- Explore opportunities to be involved in your community and develop new connections
Eligibility:This service may be right for you if you:
- Have a Developmental Disability, Autism Spectrum Disorder (ASD) or Fetal Alcohol Spectrum Disorder (FASD)
- Are between 16 and 24 years old
- Would like help planning for your future
